Participation Forms

This form allows us to publish material that may mention your cadet's name, include a photo of your cadet, or in some manner use your cadet in the presentation of personal or unit accomplishments.

This is the LAHS/LAPS Extracurricular Activities Code forms including the School Physical form. This is required for our activities and events. It also gives us the necessary information and approval to treat your cadet if something should happen during one of our events.

The NJROTC Standard Release Form is required by the Navy for all cadets participating in NJROTC.
