How to request a School Compliance Verification Form
To avoid delays, students should email Mrs. Price at least one week before the intended test date to request the School Compliance Verification Form.
Important Notes:
Mrs. Price will review attendance and grades from the most recently completed semester before issuing the form.
If testing during summer break, email before school ends and include the date you plan to take the test.
Completed forms will be placed in the front office or the pick-up box outside the school.
No Pass/No Drive Law
Students who receive their permit must remain compliant with Kentucky’s "No Pass/No Drive" law. Failure to meet academic or attendance standards may result in the suspension of the permit.
