You will receive an email through Informed K12 with the following:
Notification to follow all directions given to you by Company Nurse
If recommended to see a physician, you must complete the DWC-1 form
A reminder that it is your responsibility to provide copies of all work status notes to the Human Resources Analyst
Information on next steps if your work status has restrictions
A link to the Lammersville Workers Compensation site
Do I have to file a workers compensation claim?
If you do not wish to file a workers compensation claim and seek medical treatment, you will only fill out the first page of the Informed K12 link sent to you.
