Required documents for participation in the
KRHS Navy JROTC Program
Health Risk Screening Questionnaire: This is a required document that states your current medical conditions and limitations which may or may not prevent you from participating in NJROTC. This form also requires a sports physical, and it is required to be up to date for the duration of the school year.
NJROTC Standard Release Form: This is a required form required by the US Navy to ensure that if you are injured on a military installation while on on a field trip, the military has permission to treat you. This form is required to be up to date for the duration of the school year.
KRHS Physical Exam Form: This form is required by the Kingsway Regional High School for your participation in NJROTC.
KRHS Health History Update Form: This form will be used if you participate in one of the NJROTC activities in the spring season AND, you have a current physical on file. If you do not have a current physical on file, you will need to complete the KRHS Physical Exam Form.
NJROTC Student Code of Conduct and Parent/Guardian Consent Form: This form is new for 2024 and is a required document for maintaining enrollment in NJROTC. The purpose of this form is to share the expectations of the program, the expected behavior, and unacceptable behavior of all students and instructors taking part in the JROTC program
