Completed grade check
Pass the XLNTBrain concussion test (see directions below-cannot complete until July)

Physical Forms should be emailed to athletic director or dropped off in the KGMS front office. If having the doctor's office fax it to the school, please have them put the label "Attn: Athletic Director". ALL FOUR PAGES MUST BE COMPLETED.
