Over the past several months the KB PCN has undertaken a process to explore opportunities for optimization within our PCN. This involved in-depth conversations and engagements with PCN staff, primary care providers, and other PCN partners.

Over the course of the PCN optimization process, seven core themes emerged. The PCN Learning Lab is an opportunity to bring the network together to actively engage with these themes as we put the PCN optimization findings into practice.

The full report is a comprehesive narrative report that covers the core messages and proposed actions for the PCN as a whole as well as for each discipline. t is intended to function as a discussion document, to be reviewed by PCN partners as part of subsequent rounds of optimization engagements, and to guide our actions as we continue to work to improve our work together in primary care.
