This session focused on effective strategies for patient education in primary care. Participants explored various approaches used across the Kootenay Boundary, including interactive and personalized methods, and learned about recent contributions from the Patient Advisory Committee and Community (PACC), such as articles and surgical pathway resources.
This Learning Lab brought together Kootenay Boundary Primary Care Network (PCN) staff to explore and improve coordination of care—with a focus on Home and Community Care (HCC) and Mental Health & Substance Use (MHSU) services, as these are common and often complex points of care coordination. We examined where care coordination is working well, where it’s not, and what can be done to bridge the gaps.
This session explored the growing role of social prescribing in primary care—connecting patients to non-medical supports that improve well-being and reduce pressure on physicians. Participants reviewed existing local and provincial resources, shared examples from their own practice, and discussed gaps and opportunities to strengthen social prescribing across the region.
This interactive session explored how clinical triage can help ensure patients get the right care at the right time. Participants heard a presentation on current practices, emerging platforms, and local examples, then joined breakout discussions to share experiences and envision future models for clinical triage in their clinics and communities.
This session introduced the potential uses of AI in healthcare, featuring a presentation by Rene Perey, IH Privacy Services Manager, on Interior Health’s evolving approach to AI use and privacy considerations. Participants explored how generative AI tools like ChatGPT can support their roles and took part in breakout discussions on opportunities and concerns.
In this interactive ‘hive mind’ session we put our heads together to design a system that ensures patients find the right care at the right time. We envision what a Patient Navigator role could look like in our region, explore its potential functions, and identify the local populations that would benefit most.
Patient Navigation is an exciting innovation that aims to make primary care more equitable and accessible. This session is a dive into this topic with peers - sharing insights, and collaborating on shaping a system that works for our community.
