Entering Kindergarteners must be 5 (five) years of age on or before July 31 of the school year to enroll:
The child's birthdate must fall on or between 8/1/2020 through 7/31/2021 to enroll in kindergarten.
For Kindergarten or New Students, please complete the following:
Hard-copy Registration Packets are available from our Front Office. We also have packets available 24/7 outside on our front door.
Soft-copy Registration Packets are below to print at your convenience.
After submitting your registration, other forms and documentation must be submitted before your child can start school (Form 14/Immunization Records, TB Test dated within 12 months).

Kindergarten Registration for SY 2026-2027 begins on January 2, 2026.

Geographic Exceptions (GE) for SY2026-2027 will be accepted starting 1/2/2026.
Required Documents for GE:
Please submit the following with your GE Request Form(s):
1) A self-addressed, stamped envelope (for each request)
2) Proof of residency (1) document
Landline Phone bill, Electric bill, Water bill, Gas bill, Cable bill, Rental/Lease Agreement, Mortgage Statement, Real Property Assessment
If proof of residency is not in parent/guardian's name, a notarized Residence Verification form (see below) is required, along with an above-mentioned document in owner's name.
3) MV1 Form (Questionnaire to Determine Eligibility-- McKinney-Vento Act):

All incoming students (Gr.1-5) coming from the Mainland or International, please fill out a "Release of School Records" parent authorization form so that we may request your child's records from their previous school. Email completed form to kokoheadoffice@k12.hi.us or drop off to the main office.

Notify current DOE Hawaii School that you are withdrawing your child.
Bring completed Certificate of Release (obtained from the school you are leaving) to Koko Head Elementary.
Submit copy of one (1) Proof of Residency document (e.g., current Electric/Water/Gas/Cable/Landline-phone bill, Rental/Lease Agreement, Mortgage Statement, Real Property Assessment).
If document is not in parent/guardian's name, a notarized Residence Verification form is required, along with an above-mentioned document in owner's name.

Parents/guardians must bring the following documents and forms to enroll a child in their neighborhood school. Students who have not completed these requirements by the first day of school will not be allowed to attend school until these requirements are met.

Note: Falsification of documents submitted is subject to penalty under Hawaii Revised Statutes 710-1063, resulting in the child being sent back to the school where he/she should properly be attending. The Department may pursue prosecution at its discretion.

Tuberculosis (TB) Clearance by a U.S.-based provider dated within the past 12 months. Required prior to first day of school.

A+ Income Eligibility Table for Department of Human Services Subsidy:
Families who participate in the A+ Program may qualify for financial assistance through the Department of Human Services (DHS). To qualify, families must meet income eligibility requirements. DHS provides the A+ Programs with an Income Eligibility Table to assist families with qualifying for subsidies. For SY 2025-2026, the gross income limit for DHS A+ Program subsidies to 300% of the 2024 Federal Poverty Level for Hawaiʻi.
