For Kindergarten or New Students, please complete the following

Soft-copy Registration Packets are below to print at your convenience.

Grade K Registration Packet
Kindergarten Registration for SY 2025-2026 starts January 3, 2025. Per Hawaii State Law, entering Kindergarteners must be five years of age on or before July 31 of the school year to enter. For detailed information, click this link:

Required Docs for GE
Please submit the following with your GE Request:
A self-addressed, stamped envelope
Proof of residency
(Landline Phone bill, Electric bill, Water bill, Gas bill, Cable bill, Rental/Lease Agreement or Mortgage Statement)
Request for Records (NON-DOE)
All incoming students (Gr.1-5) coming from the Mainland or International, please fill out a "Release of School Records" parent authorization form so that we may request your child's records from their previous school. Email completed form to kokoheadoffice@k12.hi.us or drop off to the main office.
Incoming students from
(DOE Hawaii Schools)
Notify current DOE Hawaii School that you are withdrawing your child.
Bring completed Certificate of Release to Koko Head Elementary.
Bring 1 Proof of Residency (Landline phone bill, Electric bill, Water bill, Gas bill, Cable bill, Rental/Lease Agreement, Mortgage Statement)

Registration Checklist
Parents/guardians must bring the following documents and forms to enroll a child in their neighborhood school. Students who have not completed these requirements by the first day of school will not be allowed to attend school until these requirements are met.

Enrollment Form (SIS-10 Form)
Original Birth Certificate OR Passport (A copy will be made and the original will be returned to you.)
(1) Proof of Residency – Utility bill must be within the last three (3) months.
EXAMPLES: Water Bill, Electric Bill, Gas Bill, Cable Bill, Phone/Landline Bill, Mortgage Statement, Rental Agreement, Real Property Assessment ** If the above documents are not in your name, a notarized residence verification letter or Residence Verification Form is required along with the above utility bill. ** Falsification of documents submitted is subject to penalty under Hawaii Revised Statutes 710-1063, resulting in the child being sent back to the school where he/she should properly be attending. The Department may pursue prosecution at its discretion.

Kama'aina Kids has been contracted to provide our Before and After School Care. To the left is the A+ Parent Handbook for detailed information. Please note that acceptance into the program is based on space availability. Register ASAP.
