Registration for SY 2026-2027

Per Hawaii State Law: 

Entering Kindergarteners must be 5 (five) years of age on or before July 31 of the school year to enroll:

The child's birthdate must fall on or between 8/1/2020 through 7/31/2021 to enroll in kindergarten.

For Kindergarten or New Students, please complete the following:

• Hard-copy Registration Packets are available from our Front Office. We also have packets available 24/7 outside on our front door.

• Soft-copy Registration Packets are below to print at your convenience.

• After submitting your registration, other forms and documentation must be submitted before your child can start school (Form 14/Immunization Records, TB Test dated within 12 months).  

• Please submit health records to kokoheadoffice@k12.hi.us .

Registration Checklist

Parents/guardians must bring the following documents and forms to enroll a child in their neighborhood school. Students who have not completed these requirements by the first day of school will not be allowed to attend school until these requirements are met.

• Enrollment Form (SIS-10W Form)

• Original Birth Certificate OR Passport (A copy will be made.)

• Picture ID of Parent/Guardian (A copy will be made.)

• (1) Proof of Residency – Utility bill must be within the last three (3) months.

     EXAMPLES: Water/Electric/Gas/Cable/Landline-phone Bill; Mortgage Statement; Rental Agreement, Real Property Assessment.

** If the above document is not in parent/guardian's name, a notarized Residence Verification form is required, along with an above-mentioned document in the owner's name.**  

Note: Falsification of documents submitted is subject to penalty under Hawaii Revised Statutes 710-1063, resulting in the child being sent back to the school where he/she should properly be attending. The Department may pursue prosecution at its discretion.

• Home Language Survey Form

• Supplemental Kindergarten Enrollment Form (SIS-10 SKEF, for incoming kindergarteners only)

• Tuberculosis (TB) Clearance by a U.S.-based provider dated within the past 12 months. Required prior to first day of school.

• Student Health Record (Form 14) with current immunizations (to be completed by a physician’s office)

      If your child requires emergency rescue medications or other daily/routine medications, please contact the office to obtain an Emergency Rescue Medication Form.                 

• Kindergarten Questionnaire (for incoming kindergarteners only)

• Student Services Questionnaire

• School Records (if coming from out-of-state or private school)

• Certificate of Release (if coming from DOE Hawaii Schools)

• Parent Authorization for Release of School Records (if coming from out-of-state or private school)

• Legal/Court documents (if any)

• MV1 Form--Questionnaire to Determine Eligibility

• Student Publication/Audio/Video Release Form

• Technology Responsible Use Form

• Google Apps Permission Form

• Emergency Card
