Guidance Regarding School Health Forms For School Year 2025-2026
The guidance explains the four medication-related forms used in SY 2025–2026 and when each is required.
Request for Diabetes Care and Medication Administration Request for Diabetes Care and Medication Administration, SH36 DM is for students with diabetes, covering insulin, glucagon, blood glucose monitoring, and optional self-management; it must be reviewed and approved by a DOE consulting nurse.
Request to Store and Administer Medications- for all non-diabetes medications, including asthma, allergy, and seizure-related emergency meds, and also requires consulting nurse approval.
Volunteer Authorization to Administer Epinephrine and Glucagon form allows trained DOE employees or agents to administer epinephrine or glucagon in emergencies when paired with a completed SH36 or SH36 DM.
Student Medication Self-Administration and Carry Authorization allows students to carry and self-administer inhalers, epinephrine, or glucagon (if not already authorized on SH36 DM), and may prompt a consulting nurse to develop an Emergency Action Plan
