Medication Variance Reporting:
To ensure student safety all medication-related incidents must be properly documented through established reporting procedures. Medication variances encompass any deviation from standard protocols, including incorrect dosage, timing, or administration route, as well as procedural issues such as missing documentation, label discrepancies, or improper storage—regardless of whether student harm occurred. When variances are identified, staff must prioritize student safety, notify appropriate personnel including administration and consulting nurses, and coordinate parent communication as required.
All incidents must be documented using the School Health Section "Medication Variance Report" Form, with individual entries required for each occurrence.
