If your child is currently a HIDOE student and will be enrolling at Dole Middle School, please bring the following items when enrolling:
A picture ID of the Parent(s)/Legal Guardian(s) enrolling the student
Proof of Residency: TWO forms of Address Verification (i.e., Rental/Lease Agreement, Utility Bill, Housing Letter, Bank Statement)
A release packet from the previous school. Student must be released from the current public school before enrolling in another public school.
Any Legal Documents regarding custodianship of the student
