DEMOGRAPHIC CHANGE FORM (SIS-11)

If you have any updates to your current contact information or address change.  If you have a new residential address you will need to submit the Demographic Change Form (SIS-11) and a copy of your proof of residency with your name on it (Lease/Rental Agreement, Utility Bill, Housing Letter, Bank Statement) to the Dole Middle School Registrar's Office

EXCEPTION TO COMPULSORY EDUCATION FORM (Form 4140)

If you are planning to Homeschool your student, the parent/legal guardian will need to fill out the Exceptions to Compulsory Education Form (Form 4140) completely and submit it the Dole Middle School Registrar's Office.  Here is a link to the Hawaii Department of Education regarding Homeschooling.

GEOGRAPHIC EXCEPTION REQUEST FORM

If you would like to attend a school outside of the Dole Middle School Residential District, the parent/guardian will need to complete a Geographic Exception Request Form and submit it to the student's CURRENT Hawaii Public School or if a non-Hawaii Public School to submit the form to the student's RESIDENTIAL home district school along with a self-addressed stamped envelope.  Here is a link to the Hawaii Department of Education regarding Geographic Exceptions.
