Registration

There is a $65 registration fee for each student. Please pay fees in our Main Office.

OPTION 1: Parents/guardians of students coming to Joel P. Jensen Middle School from outside the Jordan School District must visit the Counseling Center in person and present the following documents:

  1. Parent/guardian ID (only a legal guardian may register a student for school)

  2. Withdrawal form from your student's previous school

  3. Proof of address (TWO pieces of mail with the legal guardian's current address and name, one of these must be a lease/rental agreement or mortgage statement)

  4. Your student's birth certificate

  5. Your student's immunization records

  6. If your student is receiving Special Education Services, a copy of their IEP

  7. Fill out the Chromebook User Agreement HERE if your child is going into 7th grade

OPTION 2: Parents/guardians of students currently enrolled in a Jordan School District school may register online using the Skyward Gradebook Family Access Portal.

  1. Navigate to www.jordandistrict.org. From the drop-down menu under 'Parents and Students' click on 'Family Access (Skyward)'

  2. Click on 'Login to Family Access (Desktops and Laptops),' or 'Family Mobile (Smartphones and Tablets)'

  3. After the 'Family Access' page opens enter your login and password in the appropriate boxes. (Note: Your password will show as ******* if you have changed it from the auto-assigned password.)

  4. Once you're logged into 'Family Access' you can view information for each of your students independently, or for all of your students at once. Use the links to navigate through the various information screens where you can view and print your student/s information.

  5. Fill out the Chromebook User Agreement HERE if your child is going into 7th grade
