Consent forms for the School-Based Health Clinic at Nicholasville Elementary will be going home next week. Please complete and sign the form and return it to school as soon as possible to enroll in the program. By returning the consent, this pre-registers your child IN CASE they need an appointment this year. Your child will not be scheduled for an appointment just because you complete the form. You will always be contacted before your child is seen for an appointment if it is needed. On the consent form, you also have the option to sign your child up for a flu vaccine if you would like them to receive one at the school clinic. Again, you will be contacted prior to this being scheduled. This clinic is not meant to replace your primary care provider if your child has one; rather, it presents a convenient option for care at the school if it is needed. If you have any questions or would like to schedule your child for an appointment, please reach out to our school nurse Melissa at Melissa.johnston@jessamine.kyschools.us
Jessamine County will be completing the SEL DESSA screener October 14th-25th which now requires parent/guardian consent. Your student will be coming home with a lime green paper giving more information on the screener. Please read about the screener and check yes, or no to consent and return it to school for your child's teacher as soon as possible.
