Volunteering as a chaperone – For all middle school parents please download this form and submit to Ms. O'Brien at JCTMS or Ms. Dills at Western Middle School. If needed ask your child to pick this form up in the front office of their school if you would like to go. All chaperones are welcome; however, you must be approved to be able to go. The school office at JCTMS can check to see if your file is current. Call 502-485-8272 during school hours.

Payment Plan/Refund – “Agreement Form”
Paperwork – All forms will be posted on Google classroom. Parents will be given a code to enter site once trip has been paid in pull. There are 8 forms to be filled out. Medical history, medications forms, contact inform, hotel room sign-ups, t-shirts, Groups, Handbook with quiz.
Room Assignments – Parents and students chose their rooms by signing up online. A link for room sign- ups will be provided the day after the last payment date. ( First come first serve!) Be sure to check website for announcements
Beds to a room – two beds to a 4 person room, no one is allowed to sleep on a floor, and roll-always are not allowed to be added to a room due to fire codes. Parents can pay extra to have a single occupant bed or private room.
Hotel Rooms – All girls or all boys. No moms with sons or dads with daughters unless you pay for a private room for the two of you or your family has 3 or 4 going. (Mom with 2 sons, Dad with daughter & son, both parents with child.) There must be at least 3 people in a room to be charged regular cost.
Medicines/Medical Forms – Every medication must have a form to administer. Medication forms must have a specific time to be given by the doctor. The doctor can provide a written letter of Medication and times. Failure to turn in required forms will prevent your child from receiving this medication. Penalty for taking medication without form is a suspension. Over the counter medications must have a doctor sign off. Prescription medication does notneed a doctor's signature.
Cell Phones – Can not be taken on trip. Please follow me on INSTAGRAM for updates and posts. Please send me a request and I will accept.
Spending Money – Recommend no more than $100, no bills higher than $20. If the student loses it, it is gone.
Chartered Bus / Bus Assignments – Boy and girls are separated by bus or sections of the bus . Only adults of a different gender can be on a bus with the opposite gender child (mom with son or dad with daughter). If we have to have boys and girls on a bus, boys sit in the front of the bus and girls are in the back.
Group Assignments/Group Size – Student choice by signing up with link on website. First come, first reserve!
Hotel Security – Included in the trip cost is the security guard we pay to walk to halls at night. I, Ms. O’Brien, personally duct tape every hotel room. If tape is off student door, guard notifies me and whomever broke the rules will be going home. There are guards who also monitor houses and lodging while overseas.
Consequences of Breaking Rules on trip – If the offense is serious enough, students will be sent home at the parents’ expense by flight or a parent will have to drive to pick them up. That student will no longer be staying/joining the group activities for the remainder of the trip.

School trips during the school year are not family trips – Only for students in that grade and chaperones who are 18 and older. Kenya, Africa is open to families.
Stamina – All students & chaperones will be walking between 6-14 miles a day. If you don’t like walking, these trips are not for you!
Air Travel – (Washington, D.C. and Kenya, Africa) all travelers get on carry-on luggage piece and one checked piece. In all carry-on luggage, no liquids greater than 3 oz. can be in your bag. If we fly Southwest, students will be able to pick their own seating assignments; however, if we are on a different carrier, seat are assign to travelers.
