Registration Paperwork Checklist for the
2024-2025 School Year.
Below you will find registration paperwork and a checklist for the 2024-2025 school year. All paperwork needs to be emailed to christy.gabrielson@jeffco.k12.co.us
Copy of your child's birth certificate
Completed registration paperwork (including statement of health signed by a physician found on page 4) **TIP: when you open the paperwork click on "open with" and then chose ZIP Extractor. This will put it into an editable pdf format**
A copy of your child's immunizations (see attached letter if you will be getting a medical or non-medical immunization exemption)
All medical paperwork received from our nursing staff if applicable to your child

Please be sure to update your child's statement of health when it expires (also included in above packet). Here is the electronic version if you need to email it to your provider. We also accept statements of health in other formats (some pediatricians have their own form). Thank you!
