***All Summer Camp Payments will be ONLINE via the PHS Webstore on/after April 28th. Registration/Payment is to be completed BEFORE beginning participation with a camp. There are NO REFUNDS or prorated rates.
***A student-athlete does not have to participate in summer camp to try out for cross country.
***MUST BE ATHLETICALLY CLEARED! See below for more information.
You must be cleared at least 48 hours BEFORE participation in any summer camps, tryouts, practices, or games/contests (NO EXCEPTIONS). Clearance can only be issued by the Athletic Department.
ATHLETIC CLEARANCE: On/After May 15th, create an account and log-in to athleticclearance.com. Start a new clearance for Portola and the 2025-26 school year. Please complete all requested fields:
Make an appointment for a Sports Physical conducted by a licensed medical professional (i.e. your primary care doctor or urgent care/minute clinic). Use the (IUSD Physical Form). Parent section MUST be completed and hand signed/dated (MM/DD/YY). Doctor’s evaluation MUST include an office stamp and signature with date. Upload this document to your account.
Medical INSURANCE is REQUIRED for participation in Portola athletics. Options for insurance if not currently covered: Myers-Stevens.com
CONFIRMATION: When you have completed your athletic clearance online, please print/sign/date the final confirmation page
Signatures MUST be hand-written or digitally signed.
Date must be in MM/DD/YY format.
UPLOAD the Confirmation page to your account, in addition to your physical form (as a separate document).
NOTE: Portola Athletics will review/verify your clearance, and you will receive a final approval message via email. Your clearance must be good for the entire school year.
