HOW TO APPLY FOR WORKING PAPERS:
Review Department of Labor Guidelines for working papers.
Click on the link (right) to print a copy of "AT-17: Application for Employment Certificate".
Read the "General Information" page carefully and fill out the form:
Part I: Completed by parents/guardians.
Part II: Prepare documents verifying age (birth certificate, driver's license, etc.)
Part III: Obtain a copy of most recent health physical (must be within the last 12 months and state that there are no limitations to work/activity). This can be completed by your school's nursing department.
Part IV: To be completed by prospective employer.
Part V: Must be signed by a school official in the main office.
Part VI: To be completed after submission by issuing official.
