be objective and write EXACTLY what happened - do not include any names other than the referral recipient
keep feelings and emotions out of the written report

The referral below is good, however, it would need follow-up from the Dean Team.

The referral below is crystal clear and would require no follow-up by the Dean Team unless requested by the teacher.
