Pregnant and Parenting Adjustment Request Form

The Title IX office ensures reasonable adjustments for students with pregnant related conditions as part of the University’s obligation under Title IX. By completing this form the student is notifying the University of their pregnancy related condition. The notification will allow the student and the Title IX Coordinator to collaborate and develop an appropriate plan for the continuation of the student’s educational progression, as well as particular challenges the student may face while pregnant or recovering from childbirth (e.g. missed classes, make-up work, etc.).
