Year 6
Canolfan Amanwy works very closely with primary schools and aims to develop a transition plan to ensure that the pupils coming up to us are prepared for the big changes moving schools involve. We begin this process by attending Annual reviews in the primary setting. Once the local authority has agreed a placement with us, we start working towards transition plans as soon as possible. We aim to provide at least 5 visits to the Canolfan as part of the transition programme. How these look will depend on the individual pupils needs and through discussion with parents and the primary school. As an example:
Day 1: Morning visit to new setting and meet the teacher.
Day 2: Afternoon visit.
Day 3: Full day/ (day to include time over lunch.) with support of TA from current setting.
Day 4: Full day/ (day to include time over lunch.) Canolfan TA support.
Day 5: Full day/ (day to include time over lunch.) Canolfan TA support.
An Information Exchange Meeting is conducted to ensure parents and professionals can discuss pupil needs before starting at Canolfan Amanwy. We will meet with you several times before your child starts with us, and you will have the opportunity to visit us with your child. We also hold an open evening in the summer term for new parents to come and ask any questions before the summer holidays and to share important information ready to start in the September.
Leavers
Pupils can stay at Canolfan Amanwy until the end of Year 13 when they are 18 or if need is identified there is the prospect of an additional year (year 14). Our leavers are well prepared for the move from school to post 16/18 provision. We have strong links with Coleg Sir Gar and the student mentor and tutors that run programs for pupils needing Entry level courses. The school funds places for our pupils to attend college link courses in the final year of school, this means they are already comfortable in the college setting, and staff know the pupils well before they start.
