All families are required to electronically review and sign the following forms prior to using a school device:
Grades Pre-K - 4 - A parent must complete the Technology Forms - Guardians on the Forms page of the parent portal.
Grades 5-12 - Both a parent/guardian AND student must complete the Technology Forms on the Forms page of the parent/student portal.
Please note that for students in grades 5-12 the parent/guardian and student must EACH LOG IN SEPARATELY to the Parent and Student Genesis Portals to complete the forms electronically.
Once logged in to the parent portal, the forms are available on the Forms page. Please click on the name of the form to open it. The form contains both the Acceptable Use Policy and the Device Usage Policy. Once you have read each policy, click the checkbox to confirm. Submit the form once both policies have been acknowledged. Each form is student specific, so a parent must electronically complete the form for all children (you can easily switch between children in the upper right corner of the screen).
If you do not have access to the Internet, please contact your school’s office to receive hard copies of all documents.
