Transition is a process that begins when a student turns 14 and may continue until age 22 for students receiving special education services. IEP teams work with students, their families and supports to create post secondary goals to prepare students for the future.
Transition Planning Includes:
Post Secondary Vision
-Post Secondary Vision parallels the Vision Statement from the IEP
-Post Secondary means attainment of a diploma or reaching the age of 22
Disability Related Needs
-Disability Related Needs align with the student’s IEP goal areas
Action Plan (Instruction, Employment, Community/ Post School Adult Living)
-Action Plan outlines the steps the school, student, and family will take to reach Post Secondary
Vision
Teams create the Action plan utilizing the Transition Planning Form ( TPF ). The Transition Planning Form is an attachment to the student's IEP developed to outline the transition planning process.
