In an effort to help all students succeed transition planning must be effective and involves:
- Student focused planning- Personal Vision Statement, Transition Planning Form, IEP goals and objectives
- Student development- Detailing student interests, strengths and preferences following a developmental sequence addressing student disability related needs in order to build the skills necessary to achieve the student's post-secondary goals/vision
- Program structures - Academics, Functional Skills, Life Skills, Vocational/ Career Exploration, Integrated Employment Education, Opportunities for Community Experiences/Community Integration, Leisure/Social
- Family and Community involvement- Family Support and Skill Generalization- Personal Care, Community support, Independent Living Skills
- Interagency Collaboration- Department of Developmental Disabilities, Massachusetts Rehabilitation Commission, Department of Mental Health
