Student based health center

Hamilton City School District has partnered with Primary Health Solutions to  be able provide healthcare services such as medical, dental, vision, and behavioral services during the school day.  The school provides transport from your student's school to Primary Health Solutions Student Based Health Clinic located at Garfield Middle.  The school also provides transportation from the clinic back to school.  The parent or legal guardian must complete a consent packet online or a paper copy and turn into your student's building school nurse.
