Action Plans

Action plans are a document that provides that school staff necessary information on how to identify symptoms & treat a medical condition such as: asthma, bee/insect stings, food allergies, special dietary considerations etc. Action plans should be submitted every year. It is recommended to have them reviewed by your student's doctor or specialists. If the treatment is a medication needed, then a "Permit to Dispense Medication Form" must also be completed. Action plans are viewable/printable below.  Action plans may also be gotten from your student's school nurse.
