Hearing & Vision sCREENINGS

The Ohio Department of Health (ODH) requires students to be screened for hearing and vision.  Your student will be screened when it is due.  This can help identify hearing and or vision problems early.  If your student does not pass the hearing or vision test a referral letter will be mailed home at a later date.  Parents or Guardians who do not wish for their student to be screened at school must sign a wavier to opt out of the screening(s) for that year.  Each school year a new wavier must be signed.  The wavier is listed below or a paper copy can be got from the school nurse.  Below is the current guidelines from the ODH for when a child should be screened. 

Hearing & Vision: Preschool, Kindergarten, 1st, 3rd, 5th, 9th, 11th. In addition, all new enrollment students will be screened despite grade level within the school year.

Vision Only: 7th
