REMINDER: Vaccine Exclusion Date is Wednesday September 3, 2025.
All medication: prescription, non-prescription, and over-the-counter medication such as Tylenol, Zyrtec, vitamins etc. must have a "Permit to Dispense Medication Form" filled out by the student's doctor and signed by the parent. There is no exceptions to this policy. The school staff is not permitted to administer any type of medication without the appropriate documentation completed.
Once the order is completed & received by the school nurse, the parent/guardian can bring the medication to their student's school. The medication will be signed in by the nurse and the parent/guardian. Please ensure the medication matches the order and the medication is not expired. If it is a prescription medication, the medication must be in the original bottle/packaging provided by the pharmacy. The nurse cannot administer a medication based off a prescription bottle only or hospital/urgent care discharge paperwork.
No medication should be brought in by the student or kept in backpacks/lockers.

Also at anytime you can have your student get one from the school nurse or office.
