The CCS discusses options for TY services with the individual and their family (July-Aug)
The individual and their family choose to pursue Self Directed Services
The CCS initiates the Matrix score request (usually after October)
The Regional Information Form (RIF) is completed and submitted to DDA (Oct/Nov)
After receipt of the RIF, DDA sends the “Ok to plan letter” to the individual with the budget amount (this is based on the individual’s matrix score)
Individual and family choose a Fiscal Management Service (FMS) and Support Broker
The CCS completes the Health Risk Screening Tool ( HRST)
The team has the waiver meeting (after January) and completes the initial plan and budget (IP & B) for Self Direction (Jan-March)
The IP & B are submitted with the Community Pathways Waiver application to DDA (usually after March)
DDA reviews the IP & B and provides a start date for services if the individual is approved for the waiver. Start dates occur after July 1st
The CCS discusses options for TY services with the individual and their family (July-Aug) and provides a list of agencies that provide the service(s) of interest
The individual and their family choose to pursue Traditional Services
The CCS encourages the individual and their family to attend the TY Fair (Oct./Nov)
The CCS communicates with the transition teacher to request documentation (if needed) including but not limited to: latest IEP, most current psychological evaluation, other assessments, behavior plans, work evaluations, etc.
The CCS initiates the Matrix score request (October) which also triggers the start of the Supports Intensity Scale (SIS) process
Individuals and families are contacting providers to learn about their programs and tour their sites (Nov/Dec). They also complete the TY Universal Application and give a copy to their CCS
The family notifies the CCS of which providers they are interested in. The CCS then follows up with the provider(s) and sends out referral packets to those agencies
The family is encouraged to have their top 2-3 providers chosen (December)
Meetings are held to apply for the Community Pathways Waiver (Jan-March)
The CCS submits the waiver application to DDA (usually March but approval has to be received from DDA first)
The selected provider agency completes the Service Funding Plan (SFP) and collects signatures from the individual and CCS. After waiver approval is received, the provider submits the SFP to DDA.
DDA will review the SFP and provide a start date for services. Start dates occur after July 1st
