Personal belief exemptions will no longer be permitted beginning January 1, 2016. Personal belief exemptions submitted before January 1, 2016, are valid until the child enters kindergarten (including transitional kindergarten) or 7th grade. Valid medical exemptions from a licensed physician are not affected and will continue to be accepted.

HBCSD Over the Counter Medication Form - Must be completed in order for the health office to distribute any medication to a student
HBCSD EPI PEN Administration and Care Plan Form - Must be completed by a physician in order for the health office to administer it to a student
HBCSD Asthma Action Plan Form - Must be completed by a physician in order for a school health plan to be developed
HBCSD Oral Health - CA law requires a dental check-up by May 31 of his/her first year in public school
