As part of the college admissions process, students are required to submit a current high school transcript. There are several ways students can request their transcript.
Electronic Transcript - Send Transcripts Directly to Colleges Electronically (including Black Common App, SLATE, SENDedu, etc.)
Log into your SchooLinks account
Go to Colleges > College Applications > Add Application > Enter College Name > Select Completed & Pending > Add Application

Mailed Transcript (Current Students) - If you need a transcript mailed for college admission/scholarships:
Give the completed form to Mrs. Shelia Johnson in the main office or via email (sjohnson@hazelwoodschools.org)
Allow 3-5 business days for transcripts to be mailed.
Transcripts are sealed and if opened by anyone other than the school, it becomes unofficial
Dual Credit/Dual Enrollment Transcripts: Due to privacy laws, EHS cannot request your college transcript. If you would like a copy of your college transcript, you will need to complete a transcript request through the National Student Clearinghouse. Please note, that most college transcripts incur a cost per transcript so you should only send your college transcript to the school you plan to attend after graduation or who requires them.
Graduates/Alumni Transcripts - Transcripts for past students
You must provide a copy of your photo identification with each transcript request and email it to transcripts@hazelwoodschools.org

Please note: We are committed to fulfilling all transcript and enrollment verification requests to the best of our ability. However, due to the age and condition of some archived records, particularly those from earlier decades, the quality of available documents may be limited. Unfortunately, we are unable to enhance or improve the legibility of these records beyond what is currently available.
To ensure efficient processing, we kindly ask that you submit only one request, as additional submissions are unnecessary. As the new school year begins and demand increases, our records team assists a large number of families each day. We appreciate your patience and understanding as we work to process each request in a timely and efficient manner.
