This year, the form looks a little different. We’ve included a full list of activities, and you can simply go through and indicate which ones your child will be attending.

A few reminders:

• Please complete one form per child

• Select all Summer Rec activities your child is registered for (do not include waitlisted activities)

We do our best to help students get to and from all of their Summer Rec activities. With over 150 students, multiple teams, and a full schedule of games, practices, and events, there are many moving parts—and occasionally last-minute changes that are beyond our control.

We love being able to support students in attending their Summer Rec activities and helping them make the most of these experiences. While we do our very best to keep up with schedules and changes, there may be times when last-minute updates or unexpected circumstances make it difficult for us to get every student to every activity. Please know that our top priority is always the safety and supervision of all students during program hours.

We’ve also created a separate Summer Rec Activity Change Form. This is different from the regular schedule change form used for program attendance. This new form is specifically for updates related to Summer Rec activities, such as:

• A game where the bus leaves at a different time than the usual practice time

• Canceled or postponed practices/games

• Overlapping activities where you need to indicate which one your child will attend

• Any other activity-specific schedule changes

A few reminders:

• Summer Rec Activity Change Forms be submitted by 6:00 PM the day prior to the scheduled activity

Not a Summer Rec Activity? Fill out this activity form for Just for Kix, Summer Band, etc. 

Important note:

• If you’re asking BASE staff to drop your child off somewhere, please make arrangements with the BASE Director ahead of time. We can’t guarantee transportation, but we’ll do our best to help when possible - Just for Kix already has transportation agreements in place and doesn't require additional arrangements with Karie.

Just for Kix: We will be walking children to the studio for their class during hours of operation. Please Note: If it is storming, alternative transportation will need to be arranged by the parents. We will do our best to communicate these changes as soon as possible.
