BASE 2026-2027 Schedule Change Forms
These forms are to let us know of a temporary schedule change from your regular schedule indicated at registration. If you would like to change your regular schedule, please contact Karie Miller.
You do not need to fill out this form if your schedule is not changing.
Schedule change forms are due by Friday at NOON of the week before.
September 1-3: Due August 28th
Closed: Friday September 4th
October 12-14: Due October 9th
Early Release Wednesday October 14th- Afternoon BASE hours 1-6pm
Closed: Thursday October 15th & Friday October 16th

Select classes your child is signed up for through Just For Kix and indicate if they need to be dropped off and picked up or just dropped off at the studio.

Help keep BASE staff in the loop about any activities your child will be attending during BASE hours; especially if staff need to help them get to/from activities or be picked up by someone else.
