BASE 25-26 Schedule Change Forms

These forms are to let us know of a temporary schedule change from your regular schedule indicated at registration. If you would like to change your regular schedule, please contact Karie Miller. 

You do not need to fill out this form if your schedule is not changing.

Schedule change forms are due by Friday at NOON of the week before.  

• February 23-27 - Due February 20th

• March 2-6 - Due February 27th

• March 9-13 - Due March 6th

• March 16-19 - Due March 13th

  • NOTE: Early Release Thursday March 19th 

  • Closed Friday March 20th & Monday March 23rd - In-service

• March 24-27 - Due March 20th

  • Closed Friday March 20th & Monday March 23rd - In-service

• March 30-April 2 - Due March 27th

  • Closed Friday April 3rd - Holiday

Activities Form

We’ve put together this form to help keep BASE staff in the loop about any activities your child will be attending during BASE hours—especially if staff need to help them get to/from activities or be picked up by someone else.

Some examples of these activities include (but aren’t limited to): sports practices, musicals, piano or guitar lessons, band, tutoring, counseling, Community Ed Activities, etc.

Just For Kix Form

Fill out this form if your child is in a Just For Kix class during hours of operation. We will be walking children to the studio for their class.

Please Note: If it is storming or below zero, alternative transportation will need to be arranged by the parents. We will do our best to communicate these changes as soon as possible.
