I would like to refer my child to be evaluated, how do I do so?

A request for evaluations must be received in writing. An email/letter detailing your concerns and your student’s information should be sent to the PPS Office.

I just received a medical diagnosis for my child from our doctor, but I do not feel they need to be evaluated for classification, what should I do?

You should reach out to your building psychologist to assist you in determining what route would be best in assuring your child is accommodated accordingly from what your doctor has diagnosed.

We are preparing to move to another district. What steps should I take to ensure I have everything the new district needs to place my child properly in their new class?

If you have not already done so, you should reach out to your current building to complete a withdrawal form. You may also reach out to the PPS department to receive a copy of your child’s IEP, if you do not currently have a copy. The new school district should have you sign a records release form when you register. They will use this to reach out to us directly to obtain any other special education records they need to determine a placement.

There is something I would like to discuss adding to my child’s IEP, who do I reach out to?

Depending on the item to be added, an amendment may be made, or a meeting may have to occur. To help you determine which, please reach out to your building psychologist.

Our family is considering moving into the Hauppauge School District. What special education programs and services are available to meet my child’s needs?

I would like to change agency/providers for my child’s CPSE services, is that possible?

How do I request a copy of my child’s IEP or other special education records?

To request a copy of your child's special education records please fill out this Google Form


Please note our department only has access to special education records and reports. Copies of report cards or transcripts should be sought by contacting an elementary main office or the middle/high school guidance departments.
