Procedure for CPSE Referral
Contact Lori Sauchelli, Registrar’s Office, to obtain a registration form to register your child with the district via email, SauchelliL@hauppauge.k12.ny.us or phone, (631) 761-8260.
With your registration packet please include a letter to the CPSE chairperson stating your concerns and requesting a CPSE 4410 evaluation.
District registrar will notify CPSE chairperson when registration is completed.
The CPSE Chairperson will call to schedule an appointment to review the process with you, obtain informed consent and choose an approved 4410 evaluator. The initial CPSE meeting will be scheduled on the day you sign consent, and you will be given the tentative date and time of the meeting.
A current physical is also a required part of the CPSE 4410 evaluation. Please provide a copy of your child's physical to evaluator. A CPSE meeting cannot take place without the physical.
CPSE chairperson will send the completed consent packet to chosen 4410 evaluator.
The 4410 Evaluator will contact you to set up evaluations.
When the 4410 evaluations are completed, you will be notified of the results by the evaluator.
At least five business days prior to the scheduled CPSE meeting, you will receive a letter informing you of the date, time & location of the initial CPSE meeting. Parents are asked to confirm attendance.
The CPSE will meet on the scheduled day to determine eligibility and to make recommendations for program/services, if needed.
If you have any questions through out the process please contact Katherine Locke via email, LockeK@hauppauge.k12.ny.us or phone at (631) 761-8251.
