Where can I find my student health forms?

Does a medical provider need to sign off on all of my health forms? 

No, not at all! A medical provider will only need to sign off on the form confirming the dates of your immunizations IF you do not already have access to your own offical copy. You can also provide that information via a form that your provider likely already uses (an immunization card or print out). 

What if I don't have Health insurance? 

Guilford College requires all students to have medical insurance, and strongly encourages our students from out-of-state to ensure they have adequate coverage locally. You can speak to our Student Health Coordinator (studenthealth@guilford.edu) if you'd like to be connected to an area insurance broker familiar with the needs of college students. 

how can I turn in my health forms?

You can bring them in person to the Milner Student Health and Counseling Center M-F, 9-5 (You can submit them in person BY APPOINTMENT ONLY DURING THE SUMMER).

Most people find it easier to email the forms to us at studenthealth@guilford.edu. It's helpful if you include "Student Health Forms (Your Name)" in the subject line to ensure they are findable. 

Forms can also be faxed to us at 336-316-2184 *note December 2023 our fax machine line is currently down*

If you are on campus for an orientation event such as GO or Spring Into Guilford - It is preferred you turn them in directly to the Student Health Services coordinator or staff member at the event on my behalf. If you decide to turn them in to anyone outside of myself, you are responsible for notifying student health and remembering who you gave them to. You can even drop them off during check-in, but getting them to us early will make your check-in go more smoothly. 

Make sure you retain a copy for your own records!

When are my health forms and required documents due? 

Health forms and required documents are due before you begin classes. Email us at studenthealth@guilford.edu if you have questions or concerns about this.

What happens if I don't submit my records?

Ultimately, you might be unenrolled from your courses, not be able to register for future classes, and not allowed to proceed with your college career. Don't let this be you--make sure you turn in your health forms! 

What happens if I'm Felling Sick or need to see a Doctor?

There are no medical professionals on staff at Guilford. Instead, we maintain a contract with Eagle Physicans Group, which operates a walk-in clinic across the street from campus (5701 W Friendly Ave) and a traditional clinic less than a mile from campus (1210 New Garden Road). 


If you have a typical complaint/symptom, Guilford's contract will cover your services (see the list of covered services here). Other services, including some lab work, sports physicals, and medication management appointments will be billed to your health insurance. Make sure to take your Guilford Student ID with you to ensure they bill the contract!

What Insurance Does Eagle Physicians accept?
