Instructions for school sponsored athletic activities:
Provide the Parent/Legal Guardian with the GESD - “Student Athletic Sports Participation Permission Form and the GESD Athletic Policies and Expectation/Athletic Contract Forms.”
Parent/Legal Guardian must complete all sections of the GESD - “Student Athletic Sports Participation Permission Form and the GESD Athletic Policies and Expectation/Athletic Contract Forms.” and return it to the Coach (Note: students cannot be allowed to participate until the completed signed forms have been returned by the parent/guardian.)
Coach will review both forms to verify that all information has been completed and signed by the parent/guardian. (Note: ALL sections must be completed and emergency contact information must be provided with at least two contacts.)
Coach will add the student to the roster approving the student to participate in the designated school sponsored athletic activity. Coach will submit a roster of player names and telephone numbers to office and to Amanda Markiewicz. Please include your name and number on the roster. Office staff will create a school messenger contact group for emergency communication. John Moritz will provide the district communications office the list for district messenger contact group.
Coach will verify student concussion training and present training to those students who have not completed the requirement through previous athletic participation.
Coach will provide a copy of the coverage for the athletic activity coverage paid for and provided by GESD No. 40 on the students that are approved to participate in the designated sport for the current school year. The “GESD-Athletic Insurance Coverage Instructions for Filing Information Sheet” will include a copy of the Student Insurance Card and insurance.
In the event of an emergency, the Coach will contact the parent and provide a copy of the claim form and the instructions on how to file a claim.
Documentation of Student Compliance with Concussion Awareness

ARS 15-341 (24) (SB 1521) mandates that schools follow the Arizona Interscholastic Association's policies/direction to provide concussion education information to students, parents and coaches. In compliance with this law all students participating in athletics in the Glendale Elementary School District shall complete the Brainbook online concussion education course. All student athletes shall complete the course prior to participation in practice or competition in any sports. This requirement will include cheerleaders.
Students, parent, coaches, teachers and administrators can take the Brainbook concussion module located at www.aiaacademy.org then click on the quick link entitled "Brainbook". Once the new window opens, they will want to click on the link in the Login box titled "Non-Student Course Link". This will allow them to take the online concussion module without registering or taking the quiz at the end of the high school student module. This will suffice for the non-high school concussion education requirement of ARS 15-341 (24) (SB 1521) mandate.
A copy of the state statue related to concussions is posted on the AlA Academy site at www.aiaacademy.orgunder "AlA Academy News" on the left side of the home page titled Arizona State Law-- Concussions (ARS 15-341).
Each school may need to provide a computer for those students who do not have home access. All students participating in district sponsored practice sessions or games must have a copy of their completion certificate in order to participate in athletics. Please keep a copy of their certificate of completion in their student file.
