Welcome to Grafton!

Are you moving to Grafton from another town and your child has special education services (IEP)?

Please follow the process below!

1. Contact your current school to complete paperwork to withdraw the student and request that records be transferred to Grafton Public Schools.

2. Contact the TEAM chair or Special Education department in your current district to inform them that you are planning on moving out of the district.

3. Contact Melissa Johnson, Preschool Team Chair, and/or the Special Education department to inform them that you are moving into Grafton.

4. Once your previous district sends the necessary documents (IEP, previous evaluations, etc), the Preschool Team chair will reach out to you to schedule a transition IEP meeting.

5. You will also need to register your child within the Grafton Public Schools. Please use the link below, click on "Click here to Access New Student Online Registration Forms", create a new account and upload the following documents:

   1. 1. 1. 1. 1. 1. 1. • Student's birth certificate (passport-if not a U.S birth certificate)

                        • Student's health records: immunization records and copy of last physical

                        • Parent's photo id: driver's license, passport, or other valid government issued ID

                        • Proof of Residency:

-Signed lease agreement, signed purchase & sales agreement, or current year real estate tax bill

-Two utility bills: electric, cable, gas, oil, or water

• Individualized Education Program (IEP): In this section, please write on a piece of paper "IEP on file" and scan/upload that document
