Proof of residency is submitted through Final Forms for processing.
All students must submit proof of residency for the current school year.
The proof of residency must list the name of the parent/guardian, clearly display the street address, and be dated after July 1 of this school year. It can be any of the following items:
1. Utility bill dated July 1 or later (gas, electric, water, cable) Cannot be a disconnection notice
2. A deed, mortgage, lease, current home owner’s or renter’s insurance declarations page, or current property tax bill. Cannot be a car insurance statement.
3. Telephone bill dated July 1 or later (for a landline telephone-cell phone bills cannot be used, Cannot be a disconnection notice
4. Copy of a paystub dated July 1 or later
5. Documents issued by an agency of the federal, state, or county government dated July 1 or later of this year. This can include dated letters from:
-Department of Job and Family Services
-Social Security Administration
-Bureau of Workers’ Compensation
6. A copy of a bank statement dated July 1 or later (Private/personal information such as bank account number or dollar amounts can be blackened out)
