Why did I create this artifact?
One of my students has been identified as requiring explicit instruction on how to interact with his peers. As a result, he has the following goals as part of his individual program plan:
Student will interact appropriately with 1-2 peers in a structured school setting.
Student will engage in cooperative play during his time at school.
Student will maintain appropriate space and boundaries when working with others.
Student will trade information with others during the school day.
In consultation with our school's behaviour intervention specialist, it was decided that a friendship survey would be the most appropriate way to assess progress in this area. The idea being that we have the student respond to questions prior to intervention and then again after instruction to determine if his answers changed.
Why Google Forms?
I was unable to find a ready made survey that focused on the topics I wanted to assess for my student. In addition, many of the questionnaires were above my students level of understanding and reading ability. It was at this point that I determined I would need to create something myself to meet our programming needs. I chose Google Forms as I knew it would be easy to assign to my student through his GNSPES account (Google Classroom or Gmail). In addition, all google products are eaily shared with others which would make it possible to collaborate with the behaviour consultant remotely since she is only physically at my school once every two weeks. I had not used Google Forms prior to creating this survey so I figured it would be a great time to learn something new!

Website Integration
Providing access to my survey through this website was perhaps the most difficult part of the entire process. Inserting the survey onto the page wasn't a problem as there are an array of insert options for building a website which include one for google forms. The challanging part was figuring out how to change the document settings so that users outside of GNSPES could take my survey without the hassle of logging into an account. How to Make a Google Form Public
The "preview" option on Google Forms and Google Sites were instrumental in helping me to create exactly what I was looking for. It took a couple tries to get the right boxes checked and unchecked. Special thanks to my husband who recieved the survey several times as a test case to see if he could open it.
The final change I wanted to make was to have the survey appear in a smaller box on my website (similar to how a google doc shows just the first page) but I finally accepted that this is not an option with the format I had used for the quiz. To make this possible I would have had to divide the quiz into smaller sections. While this would be extremely easy to do - the finished product was student driven and I had already made the decision not to create sections so left it the way it is for the purpose of my website.
