1. Direct Care Workers (DCWs) / Caregivers
Provide in-home support to AHCCCS Long-Term Care members.
Requirements
CPR/First Aid (hands-on only)
DCW Certification (Level 1 Fundamentals, Level 2 Aging, Level 2 DD if required)
DCW enrollment in AHCCCS Training Database
TB test + immunizations
Food Handler Card
DPS Fingerprint Clearance Card
Navajo Nation background check
CIB (if applicable)
Smartphone for EVV clock-ins (or landline/FOB available)
We can help applicants obtain missing requirements.
2. NEMT Transportation Drivers
Provide safe, reliable rides to AHCCCS medical appointments.
Requirements
Valid Arizona Driver License
DPS Fingerprint Clearance Card
DOT Medical Card
5-panel drug test
Clean 5-year MVR (within 30 days)
CPR/First Aid
HIPAA + Defensive Driver certification
Navajo Nation background check
CIB (if applicable)
All vehicles equipped with Samsara GPS & cameras.
We are an AHCCCS-approved training program.
Direct Care Worker Training
Level 1 – Fundamentals
Level 2 – Aging
Level 2 – Developmental Disabilities (on request)
Written & skills tests included
All scores uploaded to the AHCCCS DCW Training Database
Important:
We do not print certificates because the official proof is the AHCCCS DCW Training Database.
Students may request a completion letter, but the database record is what AHCCCS recognizes.
Training for Other Agencies
The agency must pay by company check.
Their employee must already be entered into the AHCCCS DCW Training Database before class so we can add test scores properly.

You have the right to choose your provider
If you do not pick an agency, the case manager may assign one for you
To make sure you get the agency YOU want, simply tell your case manager:
“I choose Gishey’s Patient Care LLC as my provider.”
