Gishey’s Patient Care LLC provides compassionate, dependable, and culturally respectful in-home care services for AHCCCS members throughout the Navajo Nation.
Our mission is to help elders and individuals remain safe, comfortable, and independent in their own homes while receiving the care and support they need.
We proudly serve members enrolled under:
AHCCCS American Indian Health Plan (AIHP)
AHCCCS Long-Term Care (LTC)
We understand the importance of family, tradition, and respect for elders. Our caregivers and staff work closely with clients, families, and case managers to ensure quality care and communication.
Our caregivers assist clients with daily living activities, including:
✔ Personal care (bathing, grooming, dressing)
✔ Mobility and transfer assistance
✔ Meal preparation
✔ Light housekeeping
✔ Medication reminders
✔ Companionship
✔ Respite care for family caregivers
✔ Support for aging and disability-related needs
Services are based on the client’s approved AHCCCS care plan.
If you or a loved one may need in-home care services, the first step is applying for the AHCCCS Long-Term Care (LTC) program.
Step 1 – Contact AHCCCS or ALTCS
You may begin the process by contacting:
AHCCCS / ALTCS
Your local tribal or healthcare office
Your current healthcare provider
A hospital discharge planner or case manager
They will help start the eligibility process.

We provide:

✔ Assistance completing paperwork
✔ Faxing applications directly to the AHCCCS office
✔ Navajo-language assistance for elders and families
✔ Help understanding required documents and next steps
This helps clients and families begin the process without needing to travel to an AHCCCS office.

Step 2 – Complete an Assessment
AHCCCS will schedule an evaluation to determine whether the individual qualifies for Long-Term Care services.
This may include:
Medical review
Functional assessment
Home situation review
Financial eligibility review
Step 3 – Choose Your In-Home Care Agency
Once approved, clients have the right to choose their own in-home care provider.
If you want Gishey’s Patient Care LLC, tell your case manager directly:
“I choose Gishey’s Patient Care LLC as my provider.”
Step 4 – If You Do Not Choose an Agency
If no agency is selected, the case manager may assign one based on availability.
We encourage clients and families to actively choose a provider that best fits their needs. You always have the right to request our agency or transfer services to us later.
Our In-Home Care Supervisor in White Cone provides assistance for elders and families who prefer speaking Navajo.
We can help with:
✔ Explaining LTC services in Navajo
✔ Paper applications
✔ Home visits when needed
✔ Understanding caregiver services
✔ Communication support for elders and families
Our office staff handles intake, scheduling, EVV setup, and paperwork while our supervisor focuses on helping elders and caregivers in the field.
You may transfer your in-home care services to Gishey’s Patient Care LLC at any time.
To transfer:

Notify your case manager that you choose:
“Gishey’s Patient Care LLC as my provider.”
Once approved, we will begin assigning caregivers and setting up services
All caregivers use AHCCCS-required Electronic Visit Verification (EVV) through HHA Exchange.
We support:
Mobile App clock-ins
Landline clock-ins
FOB devices for homes without phone service
Our EVV Coordinator assists caregivers and clients with setup and troubleshooting.
Many LTC members choose trusted family members as caregivers.
