- Child must be independently toilet trained
- Child must be 3 on the start of the school year
1. Register with the TOYBOX Program

- Click on: New Account Registration or Existing Account Registration
- Click on: TOYBOX or TOYBOX-GPS Employee
- Complete registration form, registration fee and submit

- Once you are registered Community Education will reach out with further instructions.

- Must be approved by MHS school nurse
- Make an appointment to get immunization paperwork approved by MHS school nurse
- Copy of Birth Certificate
- Policy Agreement
- TOYBOX Permission Slips
- Pick Up Authorization
