If a fixed asset is missing due to theft, loss, or fire, the following steps must be completed immediately by the Property Liaison Officer (PLO) and the site administrator.
Report the incident immediately to GPD.
Be prepared with the list of affected assets, including:
Asset Tag Number
Description of Item
Serial Number
Acquisition Cost (if available)
Obtain the Police Report Number for documentation.
🔥 For fire-related losses, also notify the Guam Fire Department (GFD).
Inform PMO immediately after reporting to GPD.
Submit a Certificate of Loss form to PMO for review and processing.
PMO will monitor the case and await confirmation when the police report is ready for pick-up.
PLO must notify PMO when the police report is ready for pick-up.
Once notified, PMO will retrieve the official police report.
PMO updates Tyler Munis and FAMS with disposal code TH (Theft) and marks the asset inactive until resolved.
PLO must notify both GPD and PMO immediately.
PMO will update the asset’s status in Munis and FAMS to reflect its recovery.
Use page 1 only for five (5) assets or fewer.
If more than 5 assets, continue listing in page 2 - additional listing
