We shall send the application form to your email. We will notify you of the date and time of your appointment/visit to OSA.
Accomplish and review the application form and prepare the requirements. Make sure that all ORIGINAL receipts are included in the attachment.
Once we received the documents, the application shall be forwarded to the Insurance Provider for evaluation and processing.
Applications which are incomplete or non-complaint with the insurance policy shall not be processed.

Within the same day of application, the scanned copy of the Good Moral Certificate shall be forwarded to your e-mail account.
If you opt for the hardcopy of the GMC, we shall inform you of the schedule of claiming the document.

If no ANDU Hotspot yet, visit the Network Operations and Computer Services (NOCS) to secure an account. Bring your ID or matriculation form.
Come to the campus on schedule, with ID and schedule of classes (hardcopy or digital), and proceed to OSA to register. A designated table/area shall be assigned to you.
The student shall sign on the log-sheet the following information: Name, Year and Course, Contact Number, Address, Time of Start and End of Use.
Proceed to the designated table/area.
Leave the table/area and campus after the end of online class or related task.
Remember to log in and out at the registration table.
The student shall go to OSA, and request for the gadget/s through window transaction approach.
The student shall sign on the log-sheet the following information: Name, Year and Course, Contact Number, Address, Time of Start and End of Use.
The student shall surrender his/her ID or matriculation form to the OSA staff.
The OSA staff shall issue the requested gadget/s, which shall then be inspected and acknowledged by the student.
The student shall disinfect the gadget/s.
After use, the student shall proceed to OSA to return the borrowed gadget/s. The OSA staff shall visually inspect the gadget/s.
Before the return, the student shall disinfect the gadget/s. There shall be no contact between the student and the OSA staff.
After the student leaves, the OSA shall restore the returned gadget/s.

The student shall read and accomplish the agreement form for the Borrowing of Access E-Skwela gadgets. This shall be effective in the entire second semester of School Year 2020 – 2021.
The OSA staff shall issue the requested gadget/s, which shall then be inspected and acknowledged by the student.
The student shall return the borrowed gadgets on or before the Promised Date of Return.
After use, the student shall proceed to OSA to return the borrowed gadget/s. The OSA staff shall visually inspect the gadget/s.
Before the return, the student shall disinfect the gadget/s. There shall be no contact between the student and the OSA staff.
After the student leaves, the OSA shall restore the returned gadget/s.
