Exit – For children receiving at least 6 months of special education, an Exit rating is required regardless of the exit reason (e.g. transition to kindergarten, exit from services, etc.). Exit ratings should be completed as close to the child’s exit from the program as possible. In general, exit ratings should not be completed prior to the last 60 days of program participation (i.e. not doing exit ratings in February for a child transition to kindergarten in June). If a child leaves your program unexpectedly/without notice, you should make attempts to locate the family and get an update on their status (i.e. have they move, do they not want their child participating in the program, etc.). If contact indicates that the child is withdrawing from all services or you are unable to contact a family, an exit COS should be completed if the child received 6 or more months of special education services. If it turns out that the child has moved and is known to be continuing in the program, you should communicate with the new LEA to transfer important information (e.g. IEPs, COS ratings, etc.) to ensure they are able to follow through on completing the required ratings and/or complete an interim rating for the child at the time of transfer.
