All EC teachers are required to maintain a service delivery log for the EC services they provide to students based on IEP's.
The purpose of the IEP Service Delivery Log is to document when services on the IEP have been provided. These logs will be reviewed when parents or school staff question services provided, when complaints are filed, as well as when EC records are requested for state and local audits.
Each EC Teacher will need to maintain their own log. You have the option to use either a paper or online format.

If you use the online format, retitle your log with your name and school. Then move it to your school’s compliance folder. Watch how to here.
Paper logs must include all of the same information as the online format, and they must be uploaded to your school’s Compliance Folder at the end of each grading period.
