Procedures to follow:
Once a learner has been identified, notification must be sent to the H.O.D. Guidance with a brief explanation of the problems experienced by the pupil being referred.
A meeting will be held to identify the severity of the problem. From this meeting possible solutions will be discussed. Meeting held with H.O.D. Guidance and Learner Support Committee and facilitators. The learner Support Committee will consist of:
HOD Guidance
HOD of the phase the learner is.
The Teacher of the learner.
The Grade leader
A meeting will be convened with the parents to discuss the challenges the child is experiencing in the classroom. This meeting will be organized by the teacher or the Head of Department (H.O.D.) for Guidance. If it is determined that the learner requires assessment by a psychologist or therapist, a consent form will be sent to the parent. Once the signed consent form is returned, an assessment date will be scheduled and communicated to the parent via a phone call. A detailed feedback session from the psychologist or therapist will be held, with the presence of the parents, H.O.D. Guidance, and the Teacher. All other relevant teachers will then be informed.
The appropriate teachers will develop a remedial program and homework plan to support the learner until the learner is referred to the Education Support Services (ESS) at the District Office. Progress reports will be completed at regular intervals, sent home to parents, and filed in the learner's personal records. A copy of these reports will also be placed in the learner’s support file by the H.O.D. Guidance.
LEARNERS REFERRED TO A REMEDIAL SCHOOL(Process)
1. Parents are to be notified.
2. Permission from the parents must be obtained before proceeding with the following steps.
3. Permission is typically granted during the report back session with the District Office.
4. Once permission and the LSEN number from the District Office are received, the following procedure must be followed:
Contact the relevant Remedial School to place the learner on the waiting list. Ask the parents to complete the school's application form.
Fax or send all relevant assessment reports and documents to the Remedial School.
The Remedial School will notify you when the learner is scheduled to attend. If no notification is received, maintain regular contact until all details are confirmed.
Keep a detailed record of all communications, including dates, times, and the individuals spoken to. These records should be maintained in a file or book.
5. Once all arrangements are finalized, inform the parents of the subsequent steps they need to take.
6. Maintain ongoing communication with the parents to ascertain whether the learner will return to our school or remain at the Remedial School.
LEARNERS REFERRED FOR SPECIAL EDUCATION(Process)
1. Parents are to be notified during the report back session with the District Office.
2. Permission from the parent must be obtained before any procedures can be followed.
3. Parents are required to sign a GDE 32 form to give permission for their child to attend a Special Education Class.
4. Learners under the age of 12 must be referred to a Special School, as they are too old for Special Education if they are 12 years or older.
5. Once permission has been granted by the parents and the District Office, the following procedure must be followed:
Obtain an LSEN number from the District Office.
Send copies of all reports and the signed Permission form to the new school recommended by the District Office.
Inform parents that it is their responsibility to visit the school to complete the application forms. Their child will only be placed on a waiting list once these forms are filled out.
Once the learner has been accepted at the new school, send all relevant reports, the permission form, and other pertinent documents to the new school.
Functions of each teacher involved.
Plans and organizes meetings with teachers and parents.
Refers problem cases to the Psychologist or Therapist or Guidance services.
Organize dates and times with the relevant persons for assessing of learners.
Report back to teachers on results of assessment.
Ensure that teachers are guided and assist in Remedial and Homework program.
Keeps records of all correspondence with parents, psychologists, therapist and guidance services.
Gives a detailed report of learner’s assessment and referred during the term to the Principal.
All records of interviews must be kept in the personal file. Records important observations regarding the learning in extra mural activities (within the school and outside the school. Documentation by the learner must be presented)
