    • Click on the following purple buttons to complete both packages:

      • • School Referral Package (Consent for Initial Consent, Demographics form, and Release of Information)/Formularios para el consejero escolar (Consentimiento para evaluación inicial, Información socioeconómica, y Divulgación de la información)

        • AND

        • Treatment Package (General Consents and Consents for Psychological Services)/Consentimientos para el tratamiento (Consentimientos generales de atención ambulatoria y Consentimientos para atención psicológica)

    • Once you click the appropriate button the forms will open, and the family can fill them out and sign them electronically

    • Our team will receive the forms once submitted and a referral will be initiated

    • Notify your assigned BH CM of the referral with the patient’s name
