Select ‘My Team’ > ‘Employee Relations’ > Disciplinary Actions
From the top right corner select ‘Add’
Select the Employee’s Name
Select the Disciplinary Status
● Attendance or Conduct-Performance
Select the date the Activity Date or the date that the incident occured on
Put in your ‘Description’
● For Example if this was for attendance the date and the amount of points accumulated. If this was for conduct-performance, explain the incident.
Select the Supervisor’s Name
Select the Discussion Date
● The date that this was discussed with the employee
Put in your Discussion Details
● This will be; what was discussed with the employee or a plan for improvement
Choose what type of Disciplinary Action was taken.

Final Written w/Suspension
Performance Improvement Plan

Verbal
Written
Review what was entered and select Save.
**Before HR approves the disciplinary action, Signed Disciplinary Actions as well as Supporting Documentation MUST be sent to HR via Email**
