A word from Nursing Administration:
Parents can access their child's immunization records on their own. This is their own official way of receiving this document with all the proper documentation. We prefer they do it this way too, because then you don't have to have them come in person. We cannot send immunizations via email.
How To Access Immunization Records
Use the following link {parents need to sign in to their parent portal with username/password; this will not work if using the student's portal}:

If you are unable to download your student’s immunization report and need assitance, please contact the Help Desk at 469.633.6222.
For a complete record, ask your treating physician.
From Your Own Physician
We also encourage you to ask your physician for a complete record. School records will only show the Texas Required Immunizations for School Attendance, whereas your physician's record will show ALL vaccines received.
From Texas Department of State Health Services thru ImmTrac2 (Texas Immunization Registry)
Both parents and students can request their own or their child's immunization records from ImmTrac2. Parents can request records for their children, and adults can request their own records, even if they were previously entered into the registry as minors.
Here's how to request records:
To request an immunization record for yourself or your child, fill out the Authorization to Release ImmTrac2 History form. Those who are 18 years old or older must sign an ImmTrac2 Adult Consent Form (EF11-13366). To maintain immunization records in the registry, they must submit the form by their 26th birthday.
To request a copy of a vaccine record, download the Authorization to Release ImmTrac2 History form.
Complete the form.

Once you have submitted the forms, you will receive a copy of your immunization record.

Collin County Healthcare Services
For a copy of a shot record, please fax an authorization to release form (English Request Form or Spanish Request Form) along with a valid picture ID of the person requesting the shot record to (972) 547-1812. For persons requesting their own shot record, please be aware that if the name on the picture ID does not match the information on the shot record due to marriage, legal name change, divorce, adoption, etc. additional documents such as a certified copy of a marriage certificate, birth certificate, finalized divorce decree, finalized legal name change, etc. will be required in order to process your request. Please allow up to 10 business days for the records request to be processed. Requests for copies of immunization records are $5.00.

If you are a first responder and would like to enroll in ImmTrac, please fax ImmTrac First Responder/Family Member Request Form or ImmTrac Disaster Information Retention Consent Form and current shot record to (972) 547-1811.
